The Advance Directive, consists of two parts. The Living Will is where you state the conditions under which you do or do not consent to the various critical life-saving medical treatments available. The Durable Power of Attorney for Health Care, you name an individual, known as your Agent, to manage all aspects of your health care. As in the former case, it is important for this Agent, to be available locally. The agent selected should share your fundamental philosophies as to medical treatment options. While there are few limitations in Virginia as to who you can appoint to this office, most people choose a close friend, a relative or their spouse as any medical physician is not allowed for this appointment unless they are related.

The document must be either signed with a notary public (including acknowledgment) or two adult witnesses.
