Durable Power of Attorney Oregon Form – Adobe PDF
The Oregon durable power of attorney, with which you choose an agent, sometimes referred to as the “Attorney-in-Fact”, to manage your monetary affairs for your best interests. Your Agent, besides having your complete trust, must be someone who can handle everything from paying simple bills to managing any investments or businesses.
If you choose your spouse as your agent, be aware that in Oregon, the agreement doesn’t cease upon a divorce. Therefore a revocation form will be needed. Naming an alternate or second agent in case your first choice isn’t available when needed is advisable, as is choosing candidates who will be locally available. You can repeal this form at any time by filling out the revocation form and distributing it to all institutions that may have or would accept the voided document.
