Loading...The Advance Directive for Health Care, consists of two parts. In the Living Will, or the Instruction Directive as it’s formally known in New Jersey, you state the conditions under which you would and would not wish to receive the medical procedures that you list.
Through the Power of Attorney, or formally, the Proxy Directive, you name a Health Care Agent to make all the other decisions regarding your health care. He or she must be an adult and can’t be your attending physician, nor, unless related, an employee of a facility in which reside or are a patient. Your Health Care Agent, in addition to enjoying your trust, must also ascribe to your philosophies and values as to health care, and must also be capable of defending those views in the face of others who may not agree.
- In order to make the form legal it must either be notarized or witnessed by two (2) individuals above the age of eighteen (18).
