The Advance Directive consists of two documents. A Durable Power of Attorney for Health Care and a Living Will, you set forth in writing the conditions under which you do and do not consent to any number of medical treatment and procedures.
Through the Durable Power of Attorney for Health Care, you name a person, again called you Agent, who will make all the rest of your health care decisions for however long that you can’t. The Agent should be a steady, mature individual who shares your basic philosophies as to health care, and who is able to argue for what he or she knows that you would want even in the face of opposition from strong-willed, competent people who may disagree. Again, local availability is a big plus. You can’t name your health care provider or any of the provider’s employees as your Agent.
