The Minnesota Advance Directive is split into two parts; the Health Care Power of Attorney and the Living Will. These documents is a state what types of medical care and treatment you would like at the end of your life through an agent. Under your Durable Power of Attorney for Health Care, you appoint a Health Care Agent to make all the other decisions about your health care. You will want a mature, solid individual who subscribes to your philosophies concerning health care and who will be able to advocate those values in the presence of family and medical staff. In Minnesota, your Health Care Agent can’t be anyone professionally involved in providing your health care, unless the chosen individual is a relative, spouse or domestic partner.
