Under Title 35 > Article 4 – Wyoming Health Care Decisions Act allows, in case an emergency occurs, to select a surrogate to be able to handle health care decisions on your behalf in the chance you may not be able to speak for yourself. The most common selection is a spouse, relative, or lifelong friend.
Like other power of attorney documents in Wyoming, it is recommended that the form be signed in the presence of a notary public or two (2) adult witnesses may suffice.
