A medical power of attorney, also referred to as an Advance Directive, which contains a Living Will and it is in this document that you set down the conditions under which you do and do not consent to life-extending health care procedures.
Under the Medical Power of Attorney, you choose an Agent to manage all decisions to be made in accordance with State Statute 11.94.010. Most people choose a spouse, relative or a very long-term friend as their Agent. In Washington, you are restricted from choosing most individuals involved in your health care, unless they are directly related to you. In any case, your Agent must be someone who is in close agreement with you on values as related to health care.
