The Oregon medical power of attorney, or ‘Advance Directive’, and it consists of the health care treatment directive (living will) and the durable power of attorney for health care. With the Living Will, you state unequivocally which life-saving health care procedures you do and do not consent to, and under what circumstances. You also can choose a health care representative to make all your other medical decisions while you are in a curable state of mind but incapacitated.
In Oregon, you can’t choose an agent that is a healthcare professional involved in your treatment(s) (Unless he or she is related). Your spouse, close friends, and relatives are the typical choices for this position. Under State law, this form must be witnessed by at least two persons over the age of eighteen or acknowledged by a notary.
