The North Dakota medical power of attorney form, also called a Health Care Directive is where you name an Agent to make all decisions in reference to treatment and facility options in the chance you cannot do so for yourself. The individual must be someone who shares your philosophies as to end of life treatment. Typical choices for this office are relatives, close friends or your spouse. In North Dakota, your Health Care Agent cannot be your medical provider or the provider of your long-term care. Employees of those individuals are excluded from this position unless they are related to you.
A related form that a resident may use to only choose end of life care without the use of an agent is the living will. This form should be completed and be sure to file with your primary care physician to know that if the worst should happen that your future wishes will be upheld.
