Use the revocation form to cancel any existing or power of attorney form that is currently in use whether it be for medical or financial representation. The revocation document must be notarized in order for it to become in effect. The principal should make multiple copies of the form and distribute it to the agent as well as any and all public and private institutions that may have been using the original power of attorney document.
