The Michigan power of attorney form is found in a packet called an ‘Advance Directive’ that also includes a living will. With the medical power of attorney document you designate a Patient Advocate to make all health care decisions if the day comes when you cannot make them for yourself due to incapacitation. It is highly desirable to choose an individual who will be available locally when needed, and again it is most often your spouse, a close friend or a relative who is chosen.
You and your Patient Advocate should share similar values as to health care, and he or she should be able to comfortably fill your requests when the time comes. In your living will, you set down in writing the conditions under which you will and will not consent to your end of life treatment options for medical procedures.
