It’s a wise idea to name an alternate agent, in case your first choice isn’t available if and when the time comes. If you choose to have the Power of Attorney go into effect immediately, it should be specified to be “durable”, or it terminates when your doctor declares you to be incapacitated. A more common configuration is for a “springing” durable power of attorney, which only goes into effect upon incapacitation.
